Nowhere we go at the moment requires specific training except London City, Gibralter and Innsbruck. Some places do appear in sim checks quite often though. The Vor approach onto 13l at JFK (the one with the close in turn) and Mexico city spring to mind. Many places have a written or audio visual brief which we are expected to be familiar with. That includes possible diversions both en route and at destination. So whilst most will never go there we're all pretty au fait with places like Sondestromfjord, Churchill, Irkutsk etc etc
Gib is capts only landing and required sim and a visit for capts, INN was sim for capts and Fos with a visit for captains with no restriction on who flew it. Cmf as far as I recall was just a briefing. Kai Tak was before my time but don't think it needed a sim and either pilot could fly it.
