We have been waitlisted for the BF upgrade using miles and the co-pay.
EWR-LAX is BF and requires both miles *and* co-pay? Shouldn't it be a miles only U/G.

No one knows. Variable.
Also, if we waitlist up to the day of depart[ure], do I have to do anything different or make any changes if it hits the 24-hour window and we can check in?
There has been some issue with this in the past. IIRC< you end up in this void from when the waitlist expires at 24h and when you actually then check in at the airport.
If it was me I would make sure that you are listed on the UG list (should be above plats) after check in. Check the pda site.
