The offer still exists and must be booked together with the flight. It is not possible to book Rail & Fly if you buy your ticket through the QR online portal. And it is not possible to buy Rail & Fly after ticketing.
So I either call QR reservations in Germany to buy the ticket including Rail & Fly or I use a travel agency. QR had/has good conditions for Rail & Fly, if you plan to travel by train to the airport it is the cheapest way to get there.

I think you should be able to use the Oryx Lounge at DOH. AFAIK they accept cash.
