The amount charged by the merchant is on top of the foreign transaction fee charged by your card issuer. Most US card issuers charge you 3% whenever the transaction was completed outside of the US, regardless of the currency.

(I use the Schwab/FIA Visa, which actually doesn't charge a foreign transaction fee at all, but that's besides the point.)
