One of those "elderly leisure travelers" easily could have been GS or have status and requested to use some form of upgrade instrument on the spot, especially if they were also flying standby. And, as stated, your mileage balance means nothing when it comes to upgrades, unless you decide to use some of it to "buy" the upgrade.
