The one time I've received a call from them it was of real importance regarding a very specific issue that was quickly resolved with their help.

OP, you put AA Executive Offices in quotes as if perhaps to show your uncertainty that such offices exist or that their representatives are actually decision-influencers. IME, if you're getting a call from them, you've got AA attention and they really are looking to understand your situation. This may just be about the survey, but it may also/instead be about your lack of flying this year; take advantage of the call either way.
