Since you used US Dividend Miles the question really belongs in the US forum. However, normally all taxes and fees are charged when you book the ticket. If a tax or fee is missed it can be collected at check-in, should the agent notice. (Its happened to me in Bangkok twice when Aeroplan forgot to charge the Thai Departure Tax).
