Unless there was some reason to book the flights as one-ways with separate PNR's, then, yeah, you were overcharged. A single itinerary (one-way, round-trip, or multi-city) should be $20 total. And, yes, you can combine different award types (Y/F, saver/anytime) into a single award itinerary.
The $5 is standard, booked online or on phone.
