A few quick comments :
a) There ARE still short-term visitor visas issued by Schengen member states that have limited territorial validity. These are very rare however and are only issued in cases where it is not possible to refer to the SIS due to time or technical constraits, or an adverse finding in SIS prevents the issue of a visa valid for all states but the issuing state still wishes to grant a visa.
b) A "EU resident card" is a common format document that is equivalent to the old common format "D" visa label.
c) The UK has indeed issued a few thousand common format ID cards on a trial basis. However, following the election of the new Government, these will now be considered VOID 30 days after Royal Assent is granted to the Identity Documents Bill.
