Everything from a newspaper to a child's stuffed toy counts as an extra piece of hand luggage. I've seen kids wailing at boarding gates when they are told that they either need to stuff their teddy bear into the bag (and pray the bag doesn't weigh more than 10kg now) or they won't be allowed to go on holiday. It's quite a sight.
