No, we are not likely to touch the nozzle of the bottle. Usually the bottle is opened and squeezed a little bit to expel some of the vapors from the inside of the bottle. There is really no need to take any of the liquid from the bottle.

The same procedures are applied to baby food, formula, and breast milk. I always have the person open the container so it can be tested if the lid of the container is not attached, if it is a flip top saline solution bottle, I will just do that myself, as contamination of the lid would be much more difficult. (They worry about contamination but put their child's pacifier in the bin where the person before them had their dirty shoes. ewww)
Let us consider baby food for a moment.

Containers of baby food, think strained carrots and such, are sterile until the seal is broken. After opening spoilage becomes an issue. Are you going to require a parent to open all containers of baby food for testing?

Pretty much the same for medical items. Sterile until the seal is broken.

Is it TSA's policy to contaminate sterile items to satisfy the medically exempted items testing?

I don't think I would like the liability attached to such actions.
