My first trip to Japan many years ago, I took along a set of "backpacker" heavy duty plastic cutlery but wound up not using it. More for peace of mind. I suggest just going to your local Panda Express or Geta Express, order something and take a couple extra pairs of chopsticks to practice with. Spend 20 minutes learning to use the chopsticks and you won't need a fork.

Many lower to mid-range "business" hotels have guest use coin laundry on-site. One that comes to mind is the American "Comfort Hotel" chain, which is all over Japan. Low cost, comfortable, free breakfast, free wi-fi, queen or larger beds and coin laundries. Check www.japanhotel.net or the Choice Hotels website.
