As to experience I have been flying that route for about 15 years (back to when it was a 3-class 767-200 before they retired them). Some years as often as monthly; other years 2-3 times during the year. In all that time and flights I have experienced 2 downgrades (one in each direction) so I do not think it happens often.
As a 1K I've always requested an upgrade (with miles or CR-1) and missed the upgrade only a couple of times (in the other direction, which seems a bit harder to upgrade). My 1P colleagues have mixed success clearing.
