I am in the UK, and I get points from the following
- staying at hotels
- making sure I am aware of applicable promotions
- having a HHonors Visa card
- transferring American Express points to HHonors as required
- completing surveys at e-rewards.co.uk - this is a slow way and only of marginal benefit to my total

I find that a HH Visa card offering silver status is close to an insult, seeing as there are cards in other countries offering Gold or Diamond. A Platinum Amex card in the UK offers Gold status, but only for 12 months afaik.
