I have to disagree with you.... when you serve pax and you asking them if they want drink etc and they have headphones on. You are standing less than 3 or 4 foot away from them. They nearly always have to take their headphones off for them to hear what you are saying. They are not going to hear cabin crew shouting instructions when they are 20/30 foot away.

Mr H doesn't seem to be able to hear me when he has his noise cancelling headphones on. Strange - he's taken to wearing them around the house now, even when he's not actually listening to anything...........
