Thanks everybody for the responses. I followed HereAndThereSC's advice and I think this was the problem in the past. My LH reservations have always been in the name of Mr. FirstName MiddleName LastName, whereas my UAMP account was always Mr. FirstName MiddleInitial LastName. I had UAMP change my name to match the LH reservation and it did the trick this afternoon.
I think the point is not to use your middle name. Middle names are irrelevant and will only cause problems in respect to your FFP. I know travel agents always stress to get your FULL name and the EXACT spelling but that is hysteria; I never use my middle name in flight bookings or in my FFP and have never, ever encountered a problem (and I fly a lot...)
