One of flights is operated by SAS, but for each input field I had a dropdown which allowed me to choose from multiple carriers (that is, more then the carroers which actually do my flights).

Right. So it sent the mileage info for each carrier as you entered it.
It looks like this:

United Airline: [Drop Down] Number: ___________________
Scandinavian Airline: [Drop Down] Number: ___________________
So it will send UA whatever you picked for #1, and SK whatever you picked for #2.
If you wanted CO for both, you should have entered CO for both.
