I have been through this type of experience coming home from Israel. Became very ill with food poisoning the night before flying. Almost passed out while being questioned by the airport security people in Tel Aviv. Bad timing to start sweating and turning pale during the interrogation. Fortunately, they really believed me when I said I was sick (almost demonstrated it in a very dramatic fashion). The trip home was very long. Fortunately, I was not contagious.

I got sick on the plane coming back from South Africa. Might have been food poisoning, I don't know.
As for IATA, no, thank goodness, have never used it, and am happy for that. The insurance coverage is quite good, to say the least. The only one I could have, and didn't use, was a provision for baggage loss. I think it is $100US a day, stupid Delta lost our bags(Sabena code share flight) to Brussels, had them back in time for the flight to Frankfurt(the following day). If I had gone shopping, could have been reimbursed as to a $100 US.
Oh well. The only serious loss was not being able to swim in the nice pool but I will also say, as they gave us some basic toiletries, it was nice not to have to drag those bags to/from the hotel

. Was going to a convention in Frankfurt, so lot's of bags.
