I'll try and answer what I can.

1. You need 2 eVouchers for each segment: TLV-FRA and FRA-ORD. I wasn't aware of it until I read this thread but eVouchers work on UA - if there are any seats left to upgrade to!
2. Don't know the answer. Probably not.
3. On LH longhaul all classes except heavily discounted Economy booking classes i.e. T, L, W, S.
