I have complained several times to AA, most specifically about intl premium bags at ORD. At least 4 times in the last two years I've checked in at LHR, watched them put the tags on *and* do the associated magic in the system (one agent told me it doesn't matter if the bright orange tag is on there if the magic isn't in the record), only to find my bags come off in the last *20%* of bags at ORD. One time, I even went and got an AA agent in the baggage area and had her watch with me as my bag, complete with the bright orange tag, came off almost at the very end. They always blame LHR and say that they have problems with LHR not loading the premium bags into the right location in the belly of the plane. Doesn't wash with me because I've had the same problem with several intl. originating points.
