If the parent can't control the child, for whatever reason, maybe they shouldn't be dragging him / her half way round the world on vacation.

A very good friend of mine has a child with severe ADHD. They deal with it by keeping him out of situations that they know he will not be able to deal with, like long flights! They plan their vacations around taking only flights that are short enough for him to cope with, or by driving with frequent stops along the route. He's as happy as a clam if they take a short drive to Cape Cod and he can work off his excess energy having a wonderful time - no need for 8 hour flights of misery for him and the rest of the family.
