Have you emailed them via the site using the mileage credit request? I find it to be effective so long as I can give a ticket number and city pair.
If you have the boarding passes still (always keep until posted!), the ticket number will be on them. If you don't, many credit card statements have the ticket number as well. Some travel sites (such as orbitz) keep your "past" trips on line, so you can pull the information from there as well.
