It was promised in my DM Sky Club membership package and it said the card would arrive in a separate package. Haven't received it yet though.
I received one last week for the first time. It required activation at a website on the invitation materials. I didn't do anything with it since I didn't see much of a chance I would ever use it. That benefit doesn't do much for me!
