In my work place I see it everyday!
Policies are confused with rules and regulations. A few people sit at a table and decide what should be the policy based on their personal likes and dislikes. If it lessens their work, it becomes a policy.
Most "policies" are unwritten, self-serving, illogical whims of a few people.
