1) Most of the hotels I stay at don't have a lounge, but I can't remember the last time where they failed to give me the stated benefits
2) Same
3) When I request it using the confirmed suite upgrade, very rarely otherwise. On business travel getting a suite doesn't matter, but the confirmed upgrades are nice for vacation.
4) Yes
5) You get the full benefits on PL rates, but no stay credit.
