In most European countries, they simply won't allow it. I recently contacted a Cat 6 SPG hotel in Spain regarding this matter and they said I couldn't have 2 adults and 2 kids in their standard room, but that they would be happy to move me up to a larger suite (at the larger suite rate, of course) that accommodates all four. If I only had one kid, they'd be happy to add a rollaway for 70EUR/night to the standard room.
A lot of these hotels have standard rooms that only include 2-single beds OR 1-king (which usually is the two singles strapped together). I've seldom found the 2 double bed setup like most hotels in the US.
