Which is the standard answer.

And yet, on a SAA 744-combi cargo fire, the flight crew opened the doors part way as a fire management tool - I presume they must have descended to do so in which case pressurisation may not come into play. But it is clearly possible to open the doors in some circumstances.

(The 744 didn't make it, crashing short of Mauritius that it was attempting to divert into )
