Enforcement can vary widely. Many years ago in Orlando, there was a project to add another lane to I-4, and federal dollars were involved. One of the stipulations from the feds was that the new lane had to be designated HOV, even though there was little or no desire by the DOT, local leaders, and local drivers for that requirement. So the lane was marked, signs were posted, and totally ignored.
