I have a First Class Sleeper and used it on long haul LAS-PHL-VCE in 2008 and haven't used it since. I didn't find it very comfortable while used as in the photo but I am short so had a hard time maneuvering my head into the head rest while simultaneously scooting my butt forward. It may be different for taller people. I did find it extremely comfortable used as a lumbar pillow by folding in half with even less inflation. This is shown on the website.

It takes a while to adjust it because you have to make sure you don't have too much or too little air in the pillow. Also, every time I moved around the pillow moved so I would have to place it again. It does fold up into a bag but it's kind of heavy so I wouldn't use it on anything other than long hauls.
