The "green form" is an I-94W, for the Visa Waiver Program. Did you return the departure portion upon leaving? Hopefully this won't present a problem next time you try to enter the U.S. However, if you have proof that you re-entered prior to the maximum allowable stay of the VWP (re-entry stamp to the UK, boarding pass, etc.
