I've often ridden the express trains between Switzerland and Italy, albeit in first class, including Cisalpino trains, and never had a reservation - and never had to stand.
Last summer I was on a Cisalpino that I got stuck standing Milan-Lugano,but from talking to people who do the route quite a bit it doesn't seem like a common thing.
