$392 = 2 x $157 (i.e. 2 x longhaul under 9 hours) plus 2 x $39 (i.e. 2 x inter-europe flights)

Hence the fuel surchages ex HKG appear to be correct.
TMChris, I followed your link and did some reading. You will note that the chart shown has six "footnotes" numbered 1 - 6.

Foot note #1 states: 1. For tickets purchased using credit or debit cards, this is the country where the trip starts. For tickets purchased using BA Miles (for Executive Club), this is the country in which the Member resides.

This footnote #1 is placed on the following statement near the top: The surcharge rates detailed below, apply to all tickets issued on or after 18 December 2008, with the rates being based on the flight duration and where the ticket was purchased1.

Now to me, granted I am looking for a certain answer, so please check me on this, they are saying the members country of residence (USA) determines which fee structure is charged: country where the trip starts for credit card purchase and country of memeber residence for BAEC tickets?

And after further reading, I wonder if you cash in BAEC miles, with the NA res. office in JAX Florida, will they charge the fuel surcharge that applies to tickets issued in NA or UK? The diff. is: UK Domestic/Europe: GBP 15 & Longhaul Under Nine Hours @ GBP 78 ($143 each way or $246 R/T trip plus tax & airport fees, to get to the $303.62 quoted on BA WEB site to me). The NA sumamry is as you note above.
