The friend booked online. The OP said he had to call in an change one leg. I assumed he also changed the friend's leg. There is also no indication he gave that agent the friend's credit card to pay the fees. Since he was paying the friend's fees, why would the agent not assume he was also paying the miles, if not told otherwise?
I'm really not sure what you are asking. Person A logs into aa.com, books an award reservation for himself using his miles. When person A calls to inquire about the reservation, the agent brings it up on their screen and it shows the complete reservation, including the account from which the miles are to be deducted at ticketing.

In your specific situation, when you call, you tell the agent you would rather have the miles come from your account and the agent process the request. The OP did not make such a request, so the agent acted on an assumption (which you appear to agree with) without clarifying the situation first.

That's all I'm saying, the agent made an assumption resulting in an error.

Furthermore, I don't think a customer (even an EXP or CK member) should have to know that since the agent didn't ask for the friend's credit card than of course the agent will be deducting the miles from the caller's account rather than the friend's. That IMO, is beyond ridiculous.
