The last paper ticket I got on an airline was when my travel agent booked with MH in 2005, on an itinerary from LAX-TPE-KUL-CGK. MH also called me to reconfirm the outbound flight from LAX. Today, I still get paper tickets for rail travel, when booking from the US for European trains.
