Last paper ticket in the US was on CO circa 2002. I was booked to fly Atl-IAH-EWR and they ended up having to issue my ticket to paper at the airport when they tried to rebook me. Actually ended up on NW using their bump program since my CO flt got canceled. International I had paper tickets for a *A award JFK-FRA-PRG/DUS-CPH-EWR in 2005
