I asked an FA about the tourist card, and she said the customs form and a passport was all we needed for entry. I specifically asked about additional fees for the tourist card, and again was told that my passport and the customs form was all that was needed to enter.

At immigration, we were made to pay the $10 X 2 for the tourist cards. It's $20 bucks, so not that big a deal, but I'd love to hear what others who have flown to PUJ before on US have encounered, particularly those booking through USV, to see if there's was included. After talking to two agents and an FA on the plane down, I figured we were covered for it, but I guess not. Like I said, not a big deal, but I'd like to know what the situation should have been.
I don't know if the $10 fee is a new thing, but when we flew down to PUJ in 2004, we didn't have to pay it. We also had an air+hotel all-inclusive package through US Airways Vacations.

On the side, did you get the live music welcome on arrival? I love that airport. It's so small yet lively and the outdoor feel is nice! Reminds me of Palm Springs PSP.
