Indefinite Ticket Delays - Why!?

For the third time in a row I find myself approaching the day before departure on a long haul redemption and Diamond Club has taken the miles, billed my card, but not ticketed.

Based on all the complaints about this sort of thing, it looks like there is a more serious problem at play. I am certain in all 3 cases if I did not check the booking before departure, and then ring them to argue about it (yes, argue, because there initial response is just to 'keep waiting') I would not have been able to fly. One time they were so unwilling to ensure ticketing I had to call Pre-Travel Support!

They waste so much of my time. It seems like it's sort of like travelling was in the 1970s where you have to ring the airline to reconfirm.

Does anyone know if there is some systems problem behind all this, or is it lazy people in some office some place?
