In the spa you just get the one - they stamped my boarding pass with a 'thai spa' stamp when I made the booking.
There are however other places where massage is available - I think it is THB500 for 45 mins.
In the lounge the masseurs are variable. I had two neck and shoulders - one on the way out, one on the way back. The first one was very forgettable - more like a mosquito soft landing on my shoulders.
The second one was much better by comparison, but only once or twice did she kind of hit a knot with even part of the strength required to get into it.
The foot massages looked good (there was a fellow getting one done in front of me).
So maybe take the foot massage in the lounge and go for a paid neck and shoulders and, if you want it, ask for a strong masseur.
The 30 mins timing is also quite flexible (to put it nicely). By the time they take you in, fluff around, go off to get a hot towel just before it finishes, and then give you time to put your shoes on at the end you have about 20 mins of actual massage time all up.
Still a nice way to fill in time though
