I use
MedJet Assist. Cost is something like $350 per year, no dollar limit on benefits. If you need to be hospitalized and you're over 150 miles from home, they send a private jet with medical staff to pick you up wherever you are and fly you
home, not just to the nearest reasonable hospital. They also take care of all logistics, for example ambulance from hospital to airport, coordinating communications with your doctor and local medical personnel, etc. I've never had to make a claim, but it looks great, and online reviews seem to confirm. IMO, very cheap for what you get. They also have plans for shorter duration, say a couple of weeks for a particular trip.
