Using Value of Unused Ticket

Sorry to ask a question that's been asked before, but I can't quite find the info I'm looking for (and I'm a UA newbie):

I have an unused non-ref ticket which I booked on 17 March last year, and cancelled before travel. I know that I can use the value, less $150, against a new ticket which must be for travel by 17 March this year. I understand that any residual value will then be sent to me in vouchers.

My question is, do I have to use those vouchers by any particular date? And do I have to use them for travel by me?

Further question: The ticket I cancelled was $600, and I paid and extra $150 for E+. Do I just get the $600 value (less $150) or the E+ value too?
