I took advantage of this offer. Got the 25,000 miles really quick - within a week or so of my first use of the card. I haven't spent $1000 yet, so I have not gotten the additional 15,000 miles. As I recall you must spend $1000 in the first three months to get the additional 15,000 miles.
The companion ticket deal is good anywhere in the 48 for any coach class fare, including L, U, and T. I don't believe there are any blackouts. The person flying on the companion ticket will not get miles. The person flying on the paid ticket is eligible for miles. Neither person is eligible for ANY type of upgrade.
