The worst that can happen is that the TSA ID checker will note that the child is traveling under a different name than parents - and he/she will (thinking, of course, that they are getting The Big Catch tm somebody) call the cops and accuse the parents of kidnapping or transporting a minor across state lines for some nefarious purpose.

If that happens, the outcome will NOT be good either way.
