The airline sells it as one ticket and accepts this MCT in their system so unless there is a very significant delay in the arrival of your TLV flight, they'll meet you at your arrival gate, and drive you to the gate for the SFO flight or at least the closest security checkpoint to that gate. The SFO flight crew is also aware of your arrival time in FRA. Depending on the day of the week, there will be a number of other passengers taking the same route, and the airline will hold the flight as long as possible to get them all onboard.
