I was recently awarded Premex status. I simply FAXed a letter explaining to UA that my flight patterns had changed, including my 1999 membership cards for Alaska and Northwest, along with copies of my most recent statements.

I think the trick is that they will award Premex only to folks who have equivalent status on another airline.
