I think you're still confusing initial ticketing with upgrading a ticket that's already been issued. I'm pretty sure the $20 fee you are referring to above applies if you call reservations to book a ticket using AAdvantage miles. Since you will be upgrading an already booked ticket, I don't think the $20 fee applies. You can always call and ask before you start the transaction to be sure.

Exactly correct. The $20 phone ticketing fee would not apply in this scenario because the OP is upgrading a reservation for which a ticket has already been issued. As a result, it is a REISSUE, and the fee does not apply for reissues.
Now, if the request is made within 21 days of travel, the
award fee will apply. But the award fee is not the same as the phone ticketing fee.
