What does an ID checker do if they don't think the face matches the ID?
There are certain things that change over time (hair color - hair amount in some cases, facial hair, hair styles, age, etc), and these can serve to make it difficult to match the ID to the person bearing it. If the TDC does not think that the picture reasonably matches the person bearing it, then they elevate to an STSO. I am not sure what I can disclose about the policy past that point(sorry).
