To recap what others are saying:

1. Assuming availability, one SWU / eVIP is fine for that routing if continuous.

2 and 3. You must process immigration formalities at your USA port of entry, retrieve baggage and pass through customs, recheck for your onward flights, then return airside through security, at MIA. Your feeling is correct.
