Not being able to talk to the local concierge is a problem, recently I was arranging a gift to be delivered in Australia, I'm currently in the US and have a UK issued card. The Australian concierge would not take the call, even when I was routed via the UK office (the UK concierge was closed) It took three days to arrange, Australia concierge calling the UK, UK calling me in the USA then returning the call to UK to send onto Australia.

American Express has a global identity but if you travel outside of your card issued country you do experience a drop in convenience. The concierge is not the only part of the group that has this problem, PTS is also a pain outside your home country.

What I would like to see from Amex is either the concierge/PTS of the home country to become 24H with access via direct 800 numbers, (I've personally been on hold so long for CSR on a collect-call that the phone company, AT&T, disconnected me for in-activity on the line) or access to the local concierge/PTS.
