I heard of it happening a year or two ago on a CRJ flight from SLC -- the rampers simply neglected to load the bags. There were also reportedly some unemployed rampers in SLC at the end of that day.

That's the only such event I've heard of in years of (unfortunately) flying Delta Connection. Remember that these bags are usually piled up & loaded in plain view of the pax.
