I tried MBNA/Amex first but they said it was BMIs problem. ICC said the miles would only be added at the end of the membership year which I didn't believe. So I ended back up with MBNA/Amex again telling them the story and this time they said to leave it with them. The miles appeared after 3-4 weeks of leaving it with MBNA/Amex. I didn't receive any notification, email or a call, to say it was sorted but I'd say it was MBNA/Amex that did the deed.
